Local Services: What Exactly is Case Management?
WHAT EXACTLY IS CASE MANAGEMENT?
HIV case management is a service that is free for all clients who are HIV-positive since it is funded by federal and state grants. Case managers assist their client to find and use services in their community. In Ohio, HIV case managers are licensed social workers (“LSW” means licensed social workers and “LISW” means licensed independent social workers) who keep all client information confidential, based on the guidelines of the National Association of Social Workers.Case managers use tools such as psychosocial assessments, individualized service plans, and eligibility determination forms.
This goal is to optimize client functioning by providing quality services in the most efficient and effective manner to individuals with multiple, complex needs. You might be having trouble applying for a government program. A case manager can help you fill out the paperwork for programs such as Social Security, Medicaid, or disability. Living with HIV or AIDS is not easy.
You might need to find a support group or counselor to help you and your loved ones cope with HIV. A case manager can connect you with groups or a counselor to help you deal with having HIV or AIDS.
HIV Case Managers can link individuals and families to the following services:
- Case management
- Emergency food, clothing, and shelter
- Home health care and respite care (arranged through contacts with both home care and child care facilities)
- Transportation
- Access to entitlement programs
- Housing assistance (including Caracole and Shelter Plus Care)
- Legal assistance
- Financial assistance

- Spiritual and pastoral counseling
- Support groups
- Massage therapy
- Nutritional counseling
- Buddy program
- Meals on wheels
- PAWS (Pets Are Wonderful Support)
- Psychological counseling
- Substance abuse treatment
